These terms describe use of the Corazón Salud public website and patient portal associated with Eminat Medical Center. The portal is intended to let an eligible member view available membership, household, renewal, and accounting information and submit supported requests. The membership agreement and the terms of the selected plan govern membership benefits, prices, visit fees, cancellation, and clinical services. If those documents differ from a portal summary, the controlling membership documents and confirmed account records should be reviewed with Eminat Medical Center.
Access and account security
Portal access is for a person whose identity and authority over the relevant membership have been verified. A household member may see or request changes only within the access that Eminat Medical Center has authorized for that person. Keep account access private and notify Eminat Medical Center if you believe someone has used your account without permission. A request made through the portal, including a family-member or renewal request, is not an immediate change to membership unless it is accepted and reflected in the authoritative membership record.
Membership and medical services
Corazón Salud is a medical-services membership program of Eminat Medical Center. It is not health insurance and does not replace a health insurance policy. It does not include hospitalizations or major emergencies. Plan benefits, exclusions, visit fees, and cancellation conditions depend on the selected membership documents. Clinical decisions and appointment availability remain with Eminat Medical Center. The portal is an administrative tool and is not an emergency service or a substitute for direct clinical advice.
Accounting and third-party services
Invoice numbers, amounts, official due dates, payment records, and balances displayed through the portal are based on QuickBooks data synchronized through LILLY / Native Sales. A portal view may be temporarily unavailable or marked stale while accounting data is being reconciled. A portal request does not itself authorize a payment or create an accounting invoice. QuickBooks-hosted payment options are subject to applicable payment terms. The portal does not directly collect or process card or ACH payment details.
Acceptable use and access changes
Use the portal only for your own authorized membership and household information. Do not attempt to access another person’s information, interfere with the service, or provide false information in a request. Eminat Medical Center may need to restrict access to protect account security or comply with applicable requirements. Restricting portal access does not itself cancel an underlying membership.
